PERSONALIZED FAMILY SUPPORT AND GUIDANCE

Our Case Management & Family Support Services support families during the postpartum period and early childhood years. We serve as a central point of contact for parents seeking guidance, resources, and emotional support. Through individualized intake assessments, we identify each family’s unique needs and connect them to appropriate programs and services. Referrals may include mental health support, childcare, medical services, and basic needs assistance. Our team also supports enrollment in internal programs such as peer support groups, perinatal education classes, and the Marin Diaper Bank. We assist families with applications and eligibility requirements and provide ongoing follow-up to help ensure successful connections. Bilingual Spanish and English support is available. Our approach is compassionate, culturally responsive, and trauma-informed, with a commitment to strengthening families through consistent and reliable support.
