Our case managers work one-on-one with families to assess needs, build a personalized action plan, connect with services, and provide follow-up support. Case management is voluntary, family-driven, and confidential-and your case manager is your advocate.


INDIVIDUALIZED CASE MANAGEMENT
Our individualized case management provides compassionate, one-on-one support tailored to each family’s unique needs, circumstances, and goals. We work alongside clients to understand challenges, identify priorities, coordinate appropriate services, and create practical pathways toward greater stability, confidence, and well-being throughout the postpartum journey with support.
Our team remains a consistent, supportive partner as needs evolve, helping families stay connected to services and follow through with recommended resources. Through regular communication, thoughtful coordination, and personalized guidance, we reduce barriers, strengthen support systems, and help each client feel informed, empowered, supported, and never alone.

CARE PLANNING
Care planning helps families create a clear, personalized roadmap for postpartum support. Together, we identify immediate needs, ongoing concerns, personal goals, and available strengths, then organize practical next steps. Each plan is designed to be realistic, flexible, and responsive to the changing needs of families.
We collaborate with clients and, when appropriate, their support networks to coordinate care and encourage progress. By reviewing needs regularly and adjusting plans as circumstances change, we help families access timely support, build confidence, and maintain a stronger foundation for overall recovery, connection, and well-being.

RESOURCE NAVIGATION
Resource navigation helps families identify and access services that can address practical, emotional, and social needs during the postpartum period. We listen carefully to each family’s situation, explain available options, and guide them toward relevant programs, organizations, and supports that fit their circumstances and preferences.
Finding the right resource can feel overwhelming, especially during a major life transition. Our team simplifies the process by helping families understand eligibility, next steps, and available choices. We provide guidance so clients can move forward with greater clarity, confidence, and connection to community support.
You don’t have to figure out the system alone. That’s what we’re here for: to help with applications for Medi-Cal, CalFresh, WIC, and more, housing navigation, and rental assistance information.
REFERRALS AND WARM HANDOFFS
Our referral and warm handoff services help families connect with trusted providers when additional support is needed. Rather than simply sharing contact information, we help bridge the connection, communicate relevant needs when appropriate, and support a smoother transition from our services to appropriate ongoing care.
We recognize that reaching out for help can sometimes feel difficult. Warm handoffs provide reassurance and continuity by helping families understand where to turn next. Our goal is to reduce gaps in care, strengthen connections, and help you to feel more comfortable and supported in receiving services.

SOCIAL SUPPORT SCREENING
Social support screening helps us understand the people, relationships, and community connections available to each family today. Through respectful conversations, we explore areas such as emotional support, practical assistance, isolation, and connection. This helps identify strengths and potential gaps that may influence postpartum family well-being.
Screening is designed to be supportive, confidential, and centered on each family’s experience. When needs or concerns are identified, we can discuss options and connect families with helpful resources. By recognizing social support early, we help strengthen connections and encourage healthier, more supported postpartum experiences.

CONNECTION TO COMMUNITY RESOURCES
Connecting families to community resources helps ensure they can access practical support beyond our direct services. We help identify local programs and organizations offering assistance with family needs, basic necessities, parenting support, transportation, food, housing, and other resources that can promote greater stability and well-being.
Our approach focuses on making connections accessible and responsive to each family’s circumstances. We provide guidance on available options and encourage families to engage with resources that align with their needs. Strong community connections can reduce stress, increase support, and help families feel more secure.
